Report a Patient Safety Concern
Your safety is our highest priority. If you’d like to report a patient safety concern, you can do so by:
- Visiting The Joint Commission’s “Report a Patient Safety Concern or Event” webpage.
- Sending a fax to 630-792-5636
- Mailing your report to The Office of Quality & Pt. Safety (OQPS), The Joint Commission, One Renaissance Blvd, Oakbrook Terrace, IL 60181.
Reports of patient safety concerns and events must include the health care organization’s name, street address, city and state.
