Report a Patient Safety Concern
Your safety is our highest priority. If you’d like to report a patient safety concern, you can do so by:
- Visiting www.jointcommision.org’s “Report a Patient Safety Event” page on the Joint Commission website.
- Sending a fax to 630-792-5636
- Mailing your report to The Office of Quality & Pt. Safety (OQPS), The Joint Commission, One Renaissance Blvd, Oakbrook Terrace, IL 60181.
Reports of patient safety events must include the health care organization’s name, street address, city and state.
