Report a Patient Safety Concern
Safety is our highest priority. If you’d like to report a patient safety concern, you can do so by any of the following methods:
- Call the confidential SRCH Compliance Hotline at 707-303-8916
- Email email@example.com
- To report to The Joint Commission, visit their “Report a Patient Safety Concern or Event” webpage.
- Sending a fax to Joint Commission at 630-792-5636
- Mailing your report to The Office of Quality & Pt. Safety (OQPS), The Joint Commission, One Renaissance Blvd, Oakbrook Terrace, IL 60181.
Reports to The Joint Commission of patient safety concerns and events must include the healthcare organization’s name, street address, city, and state.
