What is MyChart?
MyChart is our patient portal. It allows you to see your medical records online and contact your care team. Through MyChart, you can:
- View your health information in one place
- View and update your medications
- Look up your test results
- Send messages to your care team
- Ask for medication refills
- View your appointments
- Review and pay bills
- Obtain payment estimates
- Request appointments

How do I access MyChart on my mobile device?
Visit this page to learn more about MyChart and how to sign up.
MyChart is available through your browser on your laptop or desktop computer. It is also available for iOS and Android devices. The mobile apps contain many of the same features as the MyChart website. To install the MyChart app go to the App Store or Google Play Store and search for “MyChart.” After you have installed the app, tap “Open” or find the MyChart icon on your device and tap to open it. Then select Santa Rosa Community Health from the list of organizations.

What username should I use?
You can create your own username but it has to be unique. If someone else already has that username, you will be asked to create a different one.
What password should I use?
You should be the only person that knows your password. It should not be shared with anyone. You should create a password you can remember or use a password manager to store it.
I forgot my password. What should I do?
On the MyChart log in screen, click the Forgot Username? Or Forgot Password? link for help. If you forgot both your username and password, start with the username first. You will be prompted to answer some security questions to verify your identity so you can recover your username or password.
What if I am having trouble using the MyChart app?
You can always use the MyChart website instead. Visit mychart.org and sign in.
Can I view a family member’s medical record in MyChart?
MyChart Proxy Access allows parents, guardians, and caregivers to view the medical records of family members or others they care for — with the patient’s permission. The level of access may vary. Learn about the different levels of MyChart access on this page.
How do I send a message to my provider?
You can easily send messages to your care team through the MyChart app.
MyChart is meant for NON urgent communication. If you are having a medical emergency, call 911.
I am not able to request an appointment or send a message to a provider.
Patients can only request appointments with and message providers they have previously seen. Some providers are intentionally not on MyChart like Behavior Health providers and Dentists. If a provider is not listed and should be, please call 707-303-3600.
I need to use MyChart for multiple health centers.
You have the option to link several accounts within MyChart, but you do need to have an account with each health center.
What should I do if some of my information in MyChart is incorrect?
Your MyChart information comes directly from your electronic medical record at SRCH. Ask your provider to correct any inaccurate information at your next visit.
Where can I find my health records that are prior to 2023?
To find records prior to 2023, you can request them through MyChart, or you can fill out a Release of Information (ROI) form from the website. English ROI. Spanish ROI.
How is MyChart secure?
SRCH takes great care to ensure your health information is kept private and secure. Access to information is controlled through secure activation codes, personal usernames, and passwords. Each person controls their password, and the account must be accessed using that password. On the MyChart mobile app, you can also log in using face ID or fingerprint login. Unlike conventional email, all MyChart messaging is done while you are securely logged in to the website or mobile app.
