Now is the time to transition your care from our Pediatrics Campus to one that specializes in Adult Care.

 

Choose a New Provider
The first thing you need to do is choose a new provider at a new campus. Santa Rosa Community Health has several campuses with friendly and skilled family medicine practitioners.

Call 707-303-3600 and press 1, to schedule an appointment with a provider at one of these campuses most convenient to you:

If you decide to see a different provider not affiliated with Santa Rosa Community Health, you will need to transfer your medical records. You can read more about this process below.

Keep Your Health Insurance: Talk with an Access Navigator
Some types of pediatric health insurance will end when you turn 19. There are many free or low-cost programs for adults, but these programs are not automatic. You can make a free appointment with one of our helpful Access Navigators to ensure you stay covered. Ask at your next appointment or call today.

To speak with an Access Navigator, call 707-547-3030 they are available 8 am – 5 pm, Monday – Friday.

If you do lose your health insurance, you may qualify to pay a reduced price for some services and treatments through our “Sliding Scale.” The exact amount you pay depends on your income and family size.

Transitioning to Adult Care: What You Need to Know
The main difference between pediatric and adult healthcare is that now YOU are the only person communicating with your provider and making your healthcare decisions. If desired, you can sign a “Release of Information” (ROI) Form to give other people (like your parents) permission to access information about your medical care. However, this is not usually necessary.

Also, since you’re now the key person making decisions about your care, you are expected to manage your own appointments, medications, healthcare payments, and insurance. You will also want to be prepared with questions at your medical appointments.

Transferring Your Records
As a patient of Roseland Pediatrics, you can see any provider at any Santa Rosa Community Health location. If you choose a new provider at one of our campuses, you do not need to do anything. Your medical records will automatically transfer with you.

If you move away or decide to see a different provider not affiliated with Santa Rosa Community Health, you will need to transfer your medical records to your new provider. Complete the “Permission to Transfer Medical Records” form (located here online in English and Spanish) and mail or fax it to us at:
