Appointments at Bridge Campus are by referral only.
The Bridge Campus is co-located with Sonoma County Behavioral Health (SCBH) at their new location nicknamed “The Lakes” on Challenger Way. The clinic focuses on providing primary medical care to individuals who receive services from SCBH or are experiencing homelessness. Staff at the clinic are familiar with resources that impact these populations and are sensitive to their unique needs.
